These are launched at random on a losing spin. The Insane Asylum feature transforms all of the high-paying symbols into one high-paying symbol, and The Dungeon does similar, transforming all low pays into high pays. Things really start to get interesting when you hit 25. When this happens, you trigger the Reality Spins.

This is basically a Free Spins round where players get 3 spins. At the start of the round, players can choose one of the Reality Effects that will be active throughout the round. It is also possible to have the Walking Wild available during the round. Depending on how many symbols launched the round, there may also be a Multiplier thrown in, with a maximum of 5x being available if you managed to land 85 symbols.
